The Mental Health Resources Team (MHRT) supports older adults in the case management program with mental health needs and helps access community resources to meet their needs.

Assess Needs And Develop A Plan

The MHRT meets with you in your home to talk about your current mood, feelings, and other aspects of mental health. Then they help you develop a plan to best support your mental health.


Connect With Local Providers

The MHRT helps you connect with local counselors, therapists, psychologists, and psychiatrists covered by your insurance to establish long-term care.


peer support

A peer support specialist is someone with the lived experience of recovery from a mental health condition, substance use disorder, or both. They provide support to others experiencing similar challenges. The peer support specialists role is to assist people with finding and following their own recovery paths, without judgment, expectation, rules, or requirements.

Trainings and Presentations

The MHRT provides case manager trainings as well as presentations for senior centers on mental health topics at all Dane County Focal Points. If you are interested in having us speak, please contact us.


Referrals and Consultations

The MHRT takes referrals and consults with all Dane County Focal Points. If you are a Focal Point Case Manager and would like to consult on a client, please contact us. We are here to help.
