Case Managers work with a client and their family to assess that person’s unique needs and work with a client and their family to develop a plan of action (Individualized Service Plan) that addresses that person’s unique goals and needs.

Monitor Client’s Needs
Case Managers work with the client, their family, and other professionals to assess the client's needs and goals, connect them to the appropriate resources and provide ongoing assistance. Case managers will monitor client progress towards achieving the goals set by the client and adjust services accordingly.
Connect Client to Resources and Services
Case Managers help connect the client to resources and services to help them maintain their health, safety, independence and community connection. Examples of resources and services: Energy Assistance Food Assistance or Home-Delivered Meals, Home Chore Services, Housing Assistance, Medicare, Personal Care Services, Transportation.

Advocate for Client
Case Managers help the client communicate with their families, care networks, and other agencies and professionals. Case Managers serve as authorized representatives for specific county funded programs as needed.
